The plumber came out today found the source of the water. There was a pipe that was crimped off at some point, rather than capped. Over time, water pressure pushed through the crimp, causing the water leak. Here is the newly capped pipe:
$325 later, here's what the back of our house looks like now (yes, there is duct tape in there...don't ask. The house was built 30 years ago so who knows how it got there or how long it's been there....):
It could have been soooo much worse.
